Self Referrals - Ready to refer?
Thank you very much for reaching out to us. We know how hard this can be, and we want you to know that we are here to provide support, help, and assistance.
You can self refer by completing our online self referral form. Alternatively please download our form at the bottom of the page and follow the instructions for submission. We aim to contact you within 2-5 working days to offer an appointment to discuss the information.
If your child is under 16, you can contact your school or a teacher or GP who can fill the referral form for professionals.
If you are a young person aged 16-17 and are having difficulties filling out the self-referral form, please contact us on 0207 426 2375/2400 where appropriate and we can arrange someone to speak to you. We are open 9am and 5pm, Monday to Friday.
Our form is not be used for urgent cases that need a more immediate response. We operate a duty system between 9am and 5pm, Monday to Friday that can be contacted on 020 7426 2375. Confidentiality and consent
When you talk with a health professional in private you will expect that what is said remains confidential. This principle is central to trust between patients and their doctor or other health professionals. Details of your care and treatment, including anything you tell us, remains confidential and will only be shared within our multidisciplinary clinical team (which includes health and social care staff) if necessary.
All health professionals are bound by law and professional codes of conduct to this duty of confidentiality to their patients.
Who can support me?
We understand that filling out this form can be tough and you may need support helping you through it, you might need a family member to do it or feel more comfortable asking a professional to help you.
You can chat with your GP, school or Social Worker who can refer you directly to us via our professionals page.
If your not sure if this is service is appropriate for you, feel free to ring and have an informal chat to think about whether we might be the right service for you. We may be a able to direct you to a more appropriate service or offer some advice. Our number is 0207 426 2375/2400 and we are open Mon to Fri between 9AM to 5PM.
Useful questions to think about for a referral or assessment
Thinking about the areas below will enable us to decide how best to help your family. It may be useful for you to have some of the following questions in mind:
- What are the current concerns or problems?
- What are the triggers for seeking help at this time?
- How long has the problem existed, how severe is it, and how does it impact on the child/young person's family, education and work?
- What are the relevant psychosocial and family issues including who is in the family or important people in the kinship system? Who are the other agencies involved?
- What has been the response to previous attempts to help?
- Are the child/young person/family aware of and consenting to the referral? What is the family's motivation to seek help?
- Are there any identified risks?
- What continued involvement will you have with the family?
- Are there service access issues like language and disability?
