Referral Criteria
- Cover all schools in Newham.
- Children aged 3-19 with complex needs and requiring medical support in Nursery/School.
- Awareness/emergency care plans for medical devices (e.g., VNS, ventilation, oxygen).
- Care plans for medical devices.
- Training of school staff to manage medical devices (e.g., enteral feeding devices, tracheostomies, urinary catheters, oxygen therapy, stoma, oral/nasal suctioning, and nebulisers).
- Training of school staff to administer enteral medication and enteral feeding.
- Joint care with other teams within the Children's Community Nursing Service (e.g., Diana Team/Children's Community Nursing Team/Continuing Care Team).
Please note the exclusions for our service:
- Diabetes (Diabetes care plans are completed by the children’s diabetes team. If the child is under Newham then school should send an email to childrens.diabetes@nhs.net).

Epilepsy only (refer to PHT – see below, or Epilepsy nurse if complex).

Oncology (refer to Children’s community Nursing team elft.ccntnewham@nhs.net).

Asthma (Asthma care plans should be completed by GP or Asthma nurse specialist (if the CYP is under this service), schools are supported by the whole school asthma approach).

Shunt device care plan only – this can be referred to PHT.

Children attending school out of borough.
The Public School Health Nursing Team offer training for asthma, allergies, and epilepsy. Schools can book onto the training using links provided on the Newham Connect website here (scroll down to the Health Needs Training section).
