The London Pathway Partnership Programme was announced as the first-prize winner at the 2022 London Homelessness Awards.
The winning Inclusion Health team at Homerton Hospital is in partnership with East London NHS Foundation Trust and other community organisations, to improve care and treat the health and social care needs of people experiencing homelessness and others excluded from mainstream health services.
The Inclusion Health team aims to stop the revolving door of hospital attendance, and treat patients’ health, social care and housing needs. The team wants to bridge the gap in treatment so that all patients are able to access high quality health care, and whilst also establishing a safe and secure home, so that once the patient leaves hospital, they will be aided in their recovery and on-going health.
Irfaan Ibne, Head of Operations at East London Foundation Trust and Programme Partner, said: "Our aim is to ensure that those experiencing homelessness get the treatment and support they need. This is the most successful team collaboration across London."
The five Pathway Partnership teams based in hospitals across London include St Georges, Croydon, St Mary’s/Imperial, Homerton, and Ealing and Northwick Park.
