It will provide support for service users, relatives and carers as part of the City and Hackney Dementia Service.
The overall aim of the Navigation Service will be to ensure those who have been diagnosed with dementia can come to terms with their syndrome, reduce social isolation and continue to live their everyday lives.
The Dementia Navigation Service team will be based at the Felstead Street Clinic, the home of the City & Hackney Dementia Service.
They will be working alongside other multidisciplinary teams to provide high quality and timely care.
