What Is Transition?
Transition is the process of moving from the children’s health services into the adult health services
Why does it matter what we talk about?
In transition it matters what is discussed because, making sure you understand your condition, medication, where to go for support and your general well being will mean that you are more likely to keep your epilepsy well controlled and engage with the adult services.
