Staff within SSNT contribute to EHCP’s by providing health related information for children with complex medical needs who require interventions or management of a medical device by school staff.
Applications for EHCP’s are made through the local authority by families or professionals. When an EHCP application is made for a child or young person under SSNT, the SCYPS Educational Health Care Plan Coordinator emails the generic SSNT account and requests for the team to complete and submit a health report. The email outlines the child’s details and the deadline for submission.
All completed health reports must be emailed to elft.childhealth@nhs.net. If the child or young person is identified as not being under SSNT, staff must respond via email to the EHCP coordinator and inform them as soon as possible.
