The Palliative Care Hub team are available to provide support and advice over the phone 24 hours a day, seven days a week. When you need support or advice, you can call 01767 641 349.

A skilled member of our team will talk to you to find out what you need, and will either support you over the phone or put you in touch with an appropriate care provider.

The Palliative Care Hub team can also provide palliative care in your own home 24 hours a day, seven days a week. This may be for crisis support to help you over a particularly difficult time, or planned care for patients with continuing healthcare fast-track funding.

We provide personal care, emotional and psychological support for you and your family all planned and provided to meet your individual needs. This care is arranged in discussion with your GP, District Nurse or Specialist Nurse.

Anyone who is receiving palliative care in Bedfordshire, or anyone who is supporting a person receiving that care is entitled to use the Palliative Care Hub.

Please advise all medical/nursing staff you come in contact with that you are registered with the Palliative Care Hub.
They should contact us on 01767 641 349. This is so that we can help them provide the best possible care for you.

In order to provide and coordinate this service, we need to access your medical information. We will need permission to access this information. If we have not obtained your permission in advance, we may ask for it when you call the helpline.

The Palliative Care Hub is commissioned by the NHS Clinical Commissioning Groups in Bedfordshire and provided by Sue Ryder, in collaboration with local organisations from the health and social care sector and the voluntary sector.
