What to expect
When we receive your referral, we will telephone you and arrange for you to see us at our Nurse-Led Clinic or undertake a telephone/virtual assessment or home visit. We will assess you and ask you about your health history, your risk factors, prescribed medications, how you are doing mentally and if you have any immediate questions or queries.
If appropriate, you will then be offered a place on one of our group exercise and information programmes or on our Home Cardiac Rehab Programme.
