Diana Children's Nursing Team

The Diana Team is part of Children's Community Nursing Team (CCNT).

It is multidisciplinary and delivers community palliative care for 0 to 19 year old children and supports children with complex health needs such as those who require long-term ventilation. The focus is on respite, symptom control management and end-of-life care in order to maximise quality of life.

Any professional can refer but this must be in partnership with the Consultant or GP; Parents/Carers in the case of re-occurence of a previous condition.
