Luton Community Mental Health Hub - North
The hub provides community mental health care for service users in Luton.
It provides a coordinated model of care built around the needs of our service users and their carers, delivered through closer partnership working across health, social care and voluntary sector partners.
The hub promotes positive mental health around a recovery and strengths based model, to improve independence.
This fosters improved access to appropriate services and resources, timely transfer of care from hospital, reducing the need for hospital admission with timely handovers to partner organisations and other stakeholders.
Requests for support can be made to the hubs by health and social care professionals.
