Neighbourhood Care Team (Tower Hamlets)
The Neighbourhood Care Team consists of 12 community nurses who look after patients with nursing needs who are registered with the Jubilee Street practice.
This is a new team in Tower Hamlets with a new approach to caring for people in their own home.

There is also an evening/night service.
We provide direct nursing care and personal care such as wound management, medication management, symptom management and other specialist nursing interventions. We can advise on preventative strategies, and identify interventions and activities that will improve your psychological and physical wellbeing.
We will co-ordinate your care and with you agree key goals that we will all work towards.
Involving Others in Your Care
We will liaise with other healthcare professionals whose expertise and skills could enhance your care and rehabilitation such as physiotherapists, occupational therapists, social workers, palliative care nurses, and other specialist health professionals. We will work closely with your GP so that
everyone is up to date on the care you are receiving and your progress.
This is a nurse-led team which works closely with patient's GPs.

We welcome referrals from GPs, hospital staff, health and social care professionals, family members, and patients themselves.
Your named nurse will agree with you and your family a schedule of visits to provide care and support
