Tower Hamlets Community Health Services provides high-quality patient care and services to the residents of Tower Hamlets.
The service is provided locally for District Nursing, Physiotherapy and Occupational Therapy in four Extended Primary Care Teams. Continence, Continuing Care and Podiatry services, Frailty Assessment Clinic, Rapid Response Team, and Community Intermediate Care Team are provided Borough-wide.
TH CHS aim to ensure that the residents of Tower Hamlets are offered high-quality whole-person care, including joined-up mental health and physical health support.
All services endeavour to provide care that enables residents to recover and help in living well with long-term conditions with good quality care and as independently as possible.

To coordinate the support you receive by working closely with everyone involved in your care and treatment. This can include social services and befriending services. - Clinical psychologist
Psychologists work with colleagues to consider patients' emotional well-being and help with concerns that might get in the way of you achieving your rehab goals. - Mental Health Nurse
They can assess any concerns the patient or their family has about the patient's memory, mood or mental health and they also liaise with mental health services and the memory clinic if required.
