The Tower Hamlets Mental Health Care of Older People offers assessment, treatment, care and support for individuals and their carers who are residents of Tower Hamlets.
Our Community Mental Health Team for Older People (CMHT OP) is an integrated mental health and social care team providing assessment, intervention and treatment. The CMHT provides a high quality service by working in partnership with service users, over the age of 65 years, and their carers to promote recovery.
The team is multi-disciplinary and staffed by community mental health nurses, social workers, occupational therapists, psychologists, psychiatrists, support workers and admin staff.
The team also works in partnership with the, Diagnostic Memory Clinic, Adult Community Mental Health Teams, Tower Hamlets Adult Social Care, GPs, Alzheimer’s Society and other agencies, including the borough’s residential and nursing homes.
Our Diagnostic Memory Clinic (DMC) is a multi-disciplinary team consisting of Psychiatrists, Psychologists and Dementia Nurse Specialists. The team also has direct access to Occupational Therapists and mental health Social Workers.
The team provides a comprehensive assessment for people, of any age, who are experiencing memory problems and other cognitive difficulties to determine if they have symptoms of dementia. Where appropriate, the team are also able to offer a social care needs and carers assessment.
The team works actively in partnership with service users, carers and relatives to empower them to plan their care following diagnosis and maintain a quality of life. This includes access to evidence-based interventions, such as Cognitive Stimulation Therapy, medication to alleviate some of the symptoms of dementia and an opportunity to get involved in research.
The team also works in partnership Adult Community Mental Health Teams, Tower Hamlets Adult Social Care, GPs, Alzheimer’s Society and other agencies.

Referral is via your GP or other health or social care professional. The service currently accepts self-referrals; however, you may be referred back to your GP to ensure that we are the appropriate service to meet your needs.

We provide treatment and care for a range of mental health conditions, including Dementia, that have been longstanding or conditions that have developed more recently.
We can provide a full holistic assessment taking into account physical and psychological factors, plus any recent developments, changes or trauma.
By working in partnership with individuals and their carers we provide support and guidance, and often involve other agencies, to enable recovery through interventions that meet individual need.
We aim to provide individuals and carers with the tools they need to live independently, safely and to maintain quality of life.
