Our Extended Primary Care Team (EPCT) are divided into locality teams attached to GP clusters in the appropriate areas for Newham, Tower Hamlets and Bedfordshire. The EPCTs comprise of a diverse group of professionals including community nurses, district nurses , physiotherapists, occupational therapists, care navigators, Telehealth , rehab support workers and rapid response. The EPCT team works in close partnership with GP clusters, the Local Authority, secondary care organisations and voluntary services to meet the nursing needs of adults.
We regard it as a privilege to work in partnership with people and their families in their own home or community setting. Their care can be tailored to address the reality of their circumstances. We provide patient-centred care and help patients self-manage their symptoms to ensure that they are comfortable and attain their optimum quality of life.

Integrated Care is where it's at
A number of community nursing models are being trialled. In Tower Hamlets, the Neighbourhood Nursing team (based on the Buurtzorg model in the Netherlands) is unique in that the nurses have the freedom to plan their hours around the needs of the patient. They have no boss - and are a self-managing team and determine their own schedules and rotas, and are responsible for their own costs and training. Care packages involved other healthcare professionals such as physiotherapists, occupational therapists, social workers, palliative care nurses, and other specialist health professionals when their expertise is needed. We also have an Admission Avoidance and Discharge Service comprised of our Rapid Response Nursing Service and Discharge to Assess Team.
In Newham, community nurses are the first in the country to be able to use iPads to access patient notes, update care plans and prescribe nursing interventions, all at the touch on their finger tips.It's all happening here!
Growing and Learning at ELFT
District nurses are core members of the integrated Community Health Team. Trialling new models of care; workforce development and education is core to the success of these teams. Innovation and opportunity for staff development is key. There is a wealth of opportunity for integrated learning and professional role specific development. There is a practice development team which facilitates education initiatives for the integrated teams supporting the development of practitioners for the future to meet the changing and challenging demands of the NHS. Training and education is available in a variety of formats; we like to develop innovative ways of providing education such as simulation, role play, quizzes as well as face to face –classroom sessions and E- Learning. We promote learning on the job, as part of an integrated team learning from each other and sharing expertise is a rich and valuable resource. We encourage innovative practice and suggestions from practitioners.
