The Hubs will serve the North and South of Luton. They replace the Community Mental Health Teams (CMHTs).
The multi-disciplinary hubs provide a more coordinated model of care built around the needs of our service users and their carers, delivered through closer partnership working across health, social care and voluntary sector partners.
The change is part of the Trust’s community mental health transformation programme, developing new ways of bringing together primary and secondary care with social care, other local authority services, our third sector and local communities to support people with severe mental health needs.
The Mental Health Hubs promote positive mental health around a recovery and strengths based model, to improve independence.
This fosters improved access to appropriate services and resources, timely transfer of care from hospital, reducing the need for hospital admission with timely handovers to partner organisations and other stakeholders.
Requests for support can be made to the hubs by health and social care professionals.
