Service search
The Community Learning Disability Service is a team of health staff and social workers who assess and support people who live in Tower Hamlets with a learning disability, their families and carers.
The Community Tracheostomy Service in Tower Hamlets works in partnership with all community teams, primary care services and secondary care partners to offer specialist treatment and advise with regard to the altered airway.
We complete health assessments to determine whether patients are eligible where the NHS is responsible for arranging and providing care for all of the patient’s identified/assessed Health Care and associated Social Care needs.
THEDS is an NHS service for young people aged 16 to 25 living in the London Borough of Tower Hamlets who may have concerns about their mental health.
The team comprises of a senior nurse and/or physiotherapists and occupational therapists who will attend to any falls related referral for people age 18 and over who have fallen in their home environment.
The clinic is a borough wide service set up to provide a rapid access clinic for older people. It is a stand-alone clinic and complements Barts Health developments for the frailty pathway and frail patients within the Royal London Hospital.
Occupational Therapists, also known as OTs, work with patients who are 18 and over who may have a condition which affects their daily life and activities.

The Tower Hamlets Mental Health Liaison and Psychological Medicine (MHL) team provides a mental health assessment to people of all ages (16+) in Tower Hamlets.
