Referral Processed
Once your referral is received and processed by one of our friendly admin team members, you will be sent a text message (if permission is given) with a link to book your telephone assessment. If you do not receive a text from us after a period of time, or would prefer to speak to someone to arrange your first appointment, you can contact the service on 01234 880400.
Once your assessment is booked, you will be sent a link to the welcome pack opposite. This is useful information which we advise you to read before your appointment.
Before Assessment
As you approach the date of your assessment, you will receive a text reminder (if permission has been given) for the appointment, as well as an email (if permission is given) with a web-based version of the questionnaires above. This asks you to reflect on how you have been feeling over the previous two weeks and forms part of the assessment.
You will simply need to place a tick in the appropriate boxes and submit the questionnaire. Your answers will be automatically updated to your file ready for your assessment with the therapist.
Assessment
The therapist will call you at the date and time that you agreed, to complete the assessment. The assessment can take up to 45 minutes and you should be somewhere quiet and private to take the call and not be driving. At the end of the assessment, the therapist will agree on a date and time with you for them to call you with an outcome and to discuss the immediate options available to you.
If you are unable to attend your appointment please contact us as soon as possible so that we can offer your appointment to another patient.
Please be aware, if you do not attend (DNA) two appointments or you cancel three appointments this will result in your case is closed and you will be discharged from the service.
